I understand that, if I am offered and accept employment with Santo Dulce!, such employment will be At-Will and will be voluntary on my part and the part of my employer, and that either party can terminate the employment relationship with or without notice or cause at any time.

I also understand that the Immigration Control and Reform Act requires me to prove my eligibility to work in the United States and that my failure to provide the required proof will result in the rejection of my application, withdrawal of any offer of employment that may be extended or immediate dismissal if I become employed.

DRUG TEST NOTICE – Santo Dulce! promotes a drug-free work environment.  I understand that unless otherwise prohibited by applicable law, I may be required at any time to submit to a physical, urinalysis, or other examination as a condition of my employment with the Company, including a pre-employment drug test.By accepting employment, I agree to submit to such examinations or tests as required by the Company, all at Company expense.  Additionally, a drug test may be required following a work-related injury and prior to finalizing a promotion, unless an exception applies in your state.

I certify that the facts contained in this Employment Application are true and complete to the best of my knowledge and understand that falsified statements on this application may result in rejection of my application, withdrawal of any offer of employment that may be extended, or if employed are grounds for immediate dismissal. I understand that if I become employed, this will become part of my permanent personnel record.

I authorize investigation of all statements contained herein and the references and employers listed above to give you any and all information concerning my previous employment and any pertinent information they may have, personal or otherwise, and release the Company from all liability for any damage that may result in utilization.
